At Advanced Sports & Orthopedics, we regularly perform physicals for missionaries, scouts, and high school athletes. We also commonly perform physicals for league teams, professional entrance exams, olympic athletes, and MMA competitors.

Most insurance plans offer a FREE physical, or wellness exam, at least once a year. If you haven’t used your wellness exam in the last 12 months, ADVANCED can offer you or your child a physical/wellness exam FREE OF COST. If you are looking for a sports physical only, we have cash prices that are equal to, or cheaper than other offerings in the area.

– High school sports, dance, and cheer teams
– Junior High schools
– College Sports If your institution/team has their own forms, please bring them filled out with you to your appointment.

MISSION PHYSICAL FORM

Our Mission physicals include:
– Medical history review
– Complete exam
– Hemaglobin check (in office)
– Urinalysis (in office)
– TB Skin test (in office)
– Order for any necessary vaccinations

Please be prepared for your visit with the following:
– All immunization records
– Filled out medical history forms
– Full bladder for urinalysis.
